Your HCPOA practices a more thoughtful approach to capturing your healthcare wishes by discovering, clarifying, and communicating what is important to you in the face of serious illness. By incorporating questions directly from the American Bar Association's Toolkit for Health Care Advance Planning, we are able to move beyond surface-level answers and arrive at a richer, more precise understanding of the values, beliefs, and priorities that matter most to you.

A detailed living record of your personal values, treatment preferences, and specific medical wishes are maintained and updated. Because life is always changing, we nurture an ongoing relationship with every client through annual assessments and open, meaningful conversations that ensure your healthcare wishes evolve right alongside you. After each consultation, Your HCPOA prepares a detailed summary of the client's quality-of-life values for their review, giving clients the opportunity to confirm that every aspect of their wishes has been accurately captured. From there, clients may formalize their legal designation through their own attorney or take advantage of Your HCPOA's convenient in-house documentation process, conducted with a commissioned notary and two witnesses to ensure full legal compliance.

Going a step further, Your HCPOA personally notifies the client's primary care physician and affiliated healthcare organizations, ensuring Your HCPOA is properly documented in the client's medical chart as their designated medical emergency contact. Clients are also given Your HCPOA’s 24/7 800-number to call if needed in the event of a medical emergency.
