That is why Your HCPOA meets with you every year to revisit and document your healthcare preferences, lifestyle values, and personal wishes. These conversations are unhurried and thoughtful, giving you the space to reflect on what quality of life means to you and to update your record accordingly.
If and when the time comes of a medical emergency, serious healthcare diagnosis, or terminal illness, Your HCPOA agent will have an accurate and current picture of exactly what you want.

A key part of our Your HCPOA interview is exploring what you value most in your health mentally, physically, and spiritually. What are your goals of care not just in ideal circumstances, but in more difficult, real-world situations?
When Treatment and Values Meet
You may be asked to consider scenarios such as living with a health condition that causes daily suffering and then facing an additional medical complication. In those moments, decisions often shift from “What can be done?” to “What should be done?”
Just because medicine can do something, doesn’t mean we always should.
Questions We Explore Together

- What does a meaningful quality of life look like to you?

Continuity of Advocacy Across Specialists, Facilities, and Care Settings
Healthcare is rarely delivered in one place by one provider. As you move between specialists, hospitals, rehabilitation facilities, or home care settings, your physician-directed Your HCPOA agent travels with you.
We are available 24/7 for urgent situations and your legal Your HCPOA document is also given to your healthcare providers. You are also encouraged to share this information with your loved ones, so there are no surprises.
Your wishes, your values, and your voice remain consistent no matter where your care takes place or how many providers are involved. You are never lost in the system.
Planning for Your Daily Care Needs
During the Your HCPOA interview, we explore not only your medical wishes but also how you want to live if you need help with Activities of Daily Living (ADLs). These include everyday tasks like eating, bathing, dressing, toileting, and transferring (moving from a bed to a chair or standing up) that are essential for daily self-care.
This isn’t just short-term care, like recovering for a few weeks after surgery, but ongoing support that may become more demanding over time. Living alone may no longer be the safest option for you.
To best address these situations, we ask you: Would you prefer to remain at home with in-home support, or would a nursing home or assisted living facility be more suitable?

We also discuss your preferences for meals, personal care, mobility, and social interaction, helping Your HCPOA agent understand what makes life meaningful and comfortable for you. By clarifying these choices in advance, we can advocate for a care plan that aligns with your values, even in situations you haven’t anticipated.
Planning ahead ensures your independence, dignity, and quality of life are prioritized, so you can focus on living well, no matter what support you may need.
